In the event that my son/daughter is injured whilst playing football/travelling to and from football events and I cannot be contacted on the above number(s), I hereby give my consent for my child to receive medical attention.

By signing this from I give my consent for the club to take photos or videos of the team or individuals in it and I understand Cricklade Youth FC may use photographs of my child in its publicity material and may allow my child’s photograph to appear in the media. Cricklade Youth FC adhere to the FA guidelines to ensure these are safe and respectful and used solely for the purposes they are intended for, which are promotion and celebration of activities of the club

I have read and understood the Code of Conduct for Spectators and Parents and agree to abide by these at all times. I also agree and understand that sanctions may be imposed on me and that ultimately these may affect my child should I breach the Code of Conduct. Furthermore, I also acknowledge and agree that I am a participant for the purposes of the Football Association and Competition Rules and Regulations and therefore subject to such Rules and Regulations.

I acknowledge and agree by signing this form for the registration of my child to Cricklade Youth FC, and that I am now considered to be a Club Member and will adhere to the relevant League rules and regulations and the Club Code of Conduct for Spectators and Parents at all times.
